The information requested on this application is necessary for requesting membership in the American Chinese Crested Club, Inc. (also referred to as the Club or ACCC). Voting on this application will not take place unless it is fully complete. Applications for more than one person in a household must be separate.

Individual Membership. Individual membership is open to all residents of the United States, eighteen years of age and older who are in good standing with the American Kennel Club and who subscribe to the purposes of this Club. ($25.00) ___________
Joint Membership. Joint membership is open to two persons eighteen years of age and older residing in the same household, each are entitled to vote and hold office. A joint membership will receive one mailing of all Club publications. ($40.00 per Household) ______________
Associate Membership. Associate membership is open to all persons eighteen years and older residing outside the United States who are not United States citizens and residents of the United States who only wish to receive the Club newsletter and mailings. The dues for Associate membership will be less than the dues for Individual membership. Associate members shall not be entitled to vote or hold office or be counted toward a quorum. Two ACCC Club Members (In good standing for no less than one year) are required to be your sponsors in order for your application to be considered.
Application must be submitted within 60 days of Sponsors dated signatures.
Sponsors may sponsor no more than six member applicants in one fiscal year.
Your name included with your sponsors names, will be published in the ACCC Newsletter as an applicant. The ACCC Membership has thirty days from publication to forward in writing, to the ACCC Corresponding Secretary any viable basis for the ACCC Board to not accept your application. Once your name has been published for the 30 days and the ACCC receives no input from the ACCC Membership you name will be submitted to the ACCC Board for approval.
The ACCC is not duty-bound to offer to you reason for unapproved applications and it is understood by your submission and signature on this application.
I, THE UNDERSIGNED, hereby apply for membership in the American Chinese Crested Club, Inc., and hereby agree to abide by the Constitution and By-Laws of the American Chinese Crested Club, Inc. and by the rules and regulations of the Club and the American Kennel Club now and hereafter made. I have read and truthfully answered all questions on this application. I have read and agree to abide by the American Chinese Crested Club, Inc. Code of Ethics. I further understand that if I breach the Constitution and By-Laws and/ or the Code of Ethics would be probable cause for dismissal from the Club.
Should my application be denied, the Club is not obligated to make the reason known.
